    If you have never visited a chiropractor before, chances are you have asked yourself one question above all others:

    Is chiropractic care actually safe?

    I completely understand why people ask this question.

    The internet is full of conflicting opinions, dramatic headlines, outdated myths and personal stories that naturally make people nervous. Every week, I meet first-time patients who arrive feeling anxious because they have spent hours reading online forums or watching videos that have left them more confused than informed.     My honest answer is this:

    Chiropractic care is generally very safe when it is performed by a properly trained chiropractor who conducts a thorough clinical assessment and selects an appropriate treatment technique for the individual patient.

    Notice that I did not simply say, “Chiropractic care is safe.”

    The assessment is what makes it safe.

    That distinction is incredibly important because chiropractic treatment should never be delivered as a one-size-fits-all procedure. A responsible chiropractor must first establish what is causing the patient’s symptoms, identify possible risks and determine whether chiropractic care is appropriate at all.

    Many people imagine a chiropractic adjustment as forcefully putting bones back into place.

    That is not how modern chiropractic care works.

    Think about a rusty door hinge. The hinge has not fallen off the door. It has simply stopped moving as freely as it should.

    A chiropractic adjustment is intended to improve movement in a joint that has become stiff or restricted. When healthier movement returns, the surrounding muscles may begin to relax, irritation can decrease and the nervous system may no longer need to protect the area with as much muscle tension or spasm.

    An adjustment is a precise and controlled movement performed within the normal anatomical limits of the joint. It should never be a random, aggressive or uncontrolled force.

    Why the Assessment Is More Important Than the Adjustment

    If there is one thing I want every patient to understand, it is this:

    My first job is not to find something to adjust. My first job is to establish whether you are safe to treat.

    Before I consider any form of treatment, every patient undergoes a detailed clinical assessment. Depending on the patient and their symptoms, this may include:

    • A complete medical history
    • A discussion of current symptoms
    • A review of previous injuries and surgeries
    • A neurological examination
    • Orthopaedic testing
    • An assessment of posture and joint movement

    Only once I have a clearer understanding of what may be causing the symptoms do we discuss the available treatment options.

    This assessment also helps me recognise when a patient should be referred for imaging, further medical testing or treatment from another healthcare professional.

    Who Should Not Receive Standard Chiropractic Adjustments?

    One of the most important parts of being a safe chiropractor is recognising when a particular treatment is inappropriate.

    I would not perform a standard manual adjustment if I suspected a condition such as:

    • Cancer affecting the spine
    • Severe osteoporosis or significant bone weakness

    • Certain vascular conditions
    • A serious or progressive neurological disorder

    Referring a patient does not mean that the chiropractor has failed. It means the clinical screening process has done exactly what it was supposed to do.

    How I Keep Every Patient Safe

    Safety is not based on one decision. It is a process that begins from the moment a patient walks through the door and continues throughout their treatment plan.

    Step 1: Screen for Serious Conditions

    Before discussing treatment, I look for warning signs that suggest the pain may not be caused by a straightforward mechanical problem.

    I ask about symptoms and risk factors such as:

    • Unexplained weight loss
    • Night sweats
    • Changes in bowel or bladder function
    • Saddle numbness
    • Progressive muscle weakness
    • A previous cancer diagnosis
    • Recent trauma
    • Fever or possible infection
    • Severe pain that does not change with movement or position

    If the answers or examination findings raise concern, chiropractic treatment stops until the patient has received the necessary medical assessment.

    Step 2: Perform Neurological Testing

    The nervous system can reveal an enormous amount about what may be happening inside the body.

    • Gentle joint mobilisation
    • Soft tissue therapy
    • Traction techniques
    • Rehabilitation exercises

    The safest treatment is always the treatment that matches the individual patient.

    The Biggest Myths About Chiropractic Safety

    Myth 1: Chiropractors Put Bones Back Into Place

    This is probably one of the oldest misconceptions about chiropractic care.

    Your spinal bones are not floating around out of place. If a bone were genuinely dislocated, that would be a medical emergency.

    Chiropractic adjustments are used to improve restricted joint movement. They do not relocate dislocated bones or physically push the spine back into position.

    Myth 2: Chiropractic Adjustments Are Addictive

    Your body does not become physically addicted to chiropractic treatment.

    Some people choose to continue receiving care because they enjoy moving more comfortably or because treatment forms part of their broader approach to managing a recurring mechanical problem.

    It is similar to continuing with exercise because you appreciate the benefits.

    My goal is to improve movement, provide appropriate rehabilitation exercises and help patients become as independent as possible. Chiropractic care should not be used to make patients feel frightened of stopping treatment or dependent on endless appointments.

    Myth 3: Everyone Needs Their Neck Cracked

    Absolutely not.

    If a patient feels uncomfortable with a manual neck adjustment, we choose another technique. Consent is not something a patient gives once and then loses. You remain in control throughout the consultation and treatment.

    Many patients achieve positive results using alternatives such as:

    • Activator instrument adjustments
    • Gentle mobilisation
    • Soft tissue therapy
    • Low-force techniques
    • Gentle traction

    The crack is not the cure. The sound is simply a possible by-product of joint movement and pressure changes within the joint.

    Myth 4: Every Patient Needs X-Rays

    Modern evidence-informed practice does not support routine X-rays for every patient who visits a chiropractor.

    I recommend imaging when the patient’s history or examination findings indicate that it is clinically necessary. This may include suspected trauma, serious pathology, significant neurological changes or symptoms that require further investigation.

    A healthy patient with straightforward mechanical pain will not necessarily require an X-ray before treatment. Unnecessary imaging can expose patients to radiation without improving their treatment or clinical outcome.

    Three Patients Who Changed Their Opinion About Chiropractic Safety

    The Teacher Who Was Terrified of Neck Adjustments

    A patient once arrived physically shaking because she believed that a neck adjustment would immediately cause a stroke.

    Instead of dismissing her fears, I discussed the anatomy of the neck with her, explained the treatment options and gave her the opportunity to ask every question she had.

    Because she remained uncomfortable with manual neck adjustments, I removed them from her treatment plan entirely. We used an Activator instrument instead.

    She was amazed by how gentle and comfortable the treatment felt. There was no forceful twisting and no treatment she had not agreed to.

    What Informed Consent Really Means

    Many people think informed consent simply means signing a form before treatment.

    It does not.

    For me, informed consent is an ongoing conversation between the patient and practitioner.

    You always have the final decision. You can decline a particular technique, request an alternative or ask me to stop at any point.

    Are There Side Effects After Chiropractic Treatment?

    As with most forms of physical treatment, chiropractic care can sometimes produce temporary side effects.

    Patients may experience:

    • Immediate improvement in movement
    • A feeling of lightness or reduced tension
    • Mild muscle soreness
    • Temporary stiffness
    • Short-term fatigue

    Mild soreness can feel similar to the discomfort experienced after exercising muscles that have not been used for some time. It will often settle within 24 to 48 hours.

    Serious complications are rare, but no healthcare treatment is completely without risk. This is why careful screening, informed consent and selecting the correct technique are essential parts of responsible chiropractic care.

    If symptoms worsen significantly, new neurological symptoms develop or something feels unusual after treatment, the patient should contact their chiropractor or seek appropriate medical attention.

    The Patient Who Changed My Understanding of Safety Forever

    Looking back over my career, one patient permanently changed how I think about chiropractic safety.

    He arrived with what appeared to be straightforward upper back pain after lifting boxes. Initially, several findings suggested a simple rib joint restriction.

    But something did not fit.

    His pain remained exactly the same regardless of how he moved or positioned himself. That is unusual for a straightforward mechanical rib or muscular problem, and it immediately made me pause.

    Instead of adjusting him, I performed additional screening and checked his vital signs. His blood pressure was dangerously elevated.

    I stopped the consultation and referred him directly to the emergency department. He was experiencing an atypical cardiac event.

    A healthcare practitioner must be willing to question their first impression, investigate findings that do not make sense and recognise the limits of their own scope of practice.

    How My Approach Has Changed Over the Years

    Early in my career, I believed that bigger problems required bigger adjustments.

    Experience taught me something very different.

    Often, less is more.

    A gentle mobilisation, Activator adjustment or drop-table technique can sometimes achieve the desired clinical outcome while maximising the patient’s comfort and confidence.

    I have also learned that patients respond better when they understand what is happening and feel included in every decision. Technical skill is important, but so are communication, patience and the willingness to listen.

    Every patient deserves an individualised approach. No two people have the same medical history, pain experience, physical condition or personal comfort level.

    Questions to Ask Before Choosing a Chiropractor

    If you are concerned about chiropractic safety, asking a few questions before treatment can help you make a more informed decision.

    Consider asking:

    • Will you perform a complete assessment before treating me?
    • What do you believe is causing my symptoms?
    • Are there any warning signs or risk factors in my case?
    • Why are you recommending this particular technique?
    • Are gentler treatment options available?
    • What are the potential benefits and risks?
    • Will you refer me if my condition falls outside your scope of practice?
    • What exercises or self-management strategies can I use at home?

    A responsible chiropractor should be willing to answer these questions clearly and without making you feel rushed or pressured.

    What I Would Tell My Own Family

    If my spouse, parents or children asked whether chiropractic care was safe, I would answer without hesitation.

    Yes, provided they were seeing a properly qualified chiropractor who values the examination more than the adjustment, listens carefully, explains everything clearly and knows when not to treat.

    That is the type of practitioner I would trust with the people I love most.

    Final Thoughts

    If there is one sentence I hope every patient remembers, it is this:

    A safe chiropractor is defined not only by the adjustments they perform, but by the thorough clinical assessment they conduct before they ever touch your spine.

    Safety begins with listening.

    It continues with a careful examination.
